Our sliding fee program is a healthcare assistance program that helps you afford the care you need. Your ability to pay should not stop you from receiving the health care you need. The program utilizes a sliding scale based on your income and family size. Those eligible for the sliding fee program may receive free or reduced healthcare services, depending on your individual situation.
Community Health Northwest Florida offers this sliding fee program as a Federally Qualified Health Center (FQHC). Our local, regional, state, and federal funding sources help to offset the costs of providing care to those most in need.
