The Increased Community Services program allows eligible individuals in nursing facilities to return to their homes and communities and receive specific waiver and Medicaid services, as long as it is cost-neutral.

Individuals must be 18 years of age or older, reside in a nursing facility for at least 90 days prior to application and meet eligibility guidelines before transitioning
to the community. Individuals currently receiving services through the Home and Community-Based Options Waiver whose income exceeds the income eligibility threshold by no more than five percent may also apply.
Individuals living in nursing facilities with incomes above 300 percent of Supplemental Security Income may move into the community while also permitting them to keep income up to 300 percent of Social Security Income.
Individuals must meet nursing facility level of care.
